Home Nursing, Home Care and Medical Escorts for Punggol Families

HDB flats at sunset beside Parc Centros and a side road in Punggol

Most home nursing Punggol families receive is arranged through the Agency for Integrated Care (AIC): a hospital or polyclinic doctor applies for you, and AIC matches a provider who visits your home. Home Nursing Foundation also accepts self-referrals. Call AIC on 1800 650 6060 to start. In an emergency, call 995.

Caring for someone at home is a big job. You do not have to work out the system on your own.

This page is part of eduKate Punggol’s Essential Information for Punggol, our quick answers and where-to-go lists for everyday life in Punggol. It covers home nursing, community nurses, hospital care at home and medical escorts to appointments, with the costs, subsidies and grants that help pay for them.

Quick answer

  • Start here: ask the doctor at the hospital or Punggol Polyclinic to submit an online application to AIC, or call AIC on 1800 650 6060.
  • Home nursing: for people who are housebound, covering wound dressing, injections, feeding tubes, medication and caregiver training. From $62 a visit before subsidies.
  • Medical escorts: AIC’s Medical Escort and Transport service takes people to appointments in a wheelchair-accessible vehicle. From $40 per round trip before subsidies.
  • Community nurses: SKH’s team serves Sengkang and Punggol, but you need a GP or social service referral.
  • Help with costs: means-tested subsidies, plus the Home Caregiving Grant of up to $600 a month.

Home Nursing Punggol Families Can Use

Home care happens at your home, so a provider’s office does not need to be in Punggol. We could not verify a home-nursing provider with an office in Punggol itself; the services below serve the area or are where referrals start.

Which Kind of Help Fits

Someone is about to leave hospital

This is the best moment to arrange help. Ask the ward doctor or medical social worker to apply for home nursing or medical escorts before discharge. If your family member is at SKH and suitable, ask about SKH@Home too.

A housebound parent needs nursing tasks

AIC’s home nursing covers vital signs, medication management, injections, wound dressing, feeding tubes, care coordination and caregiver training. Apply through a polyclinic or hospital doctor, or contact HNF directly to self-refer.

Getting to appointments is the hard part

AIC’s Medical Escort and Transport service is for people who have difficulty moving on their own and have no caregiver, or whose caregiver cannot help. An escort can go with them and help them find their way around the hospital. For other accessible travel, see our guide to wheelchair access on Punggol trains.

You would like a nurse to keep an eye on things

SKH’s Community Nurses see Singapore Citizens and PRs living in Sengkang and Punggol at health posts, by video, or at home if they are homebound. You cannot book them yourself: your GP or a social service partner refers you, and the team contacts you within three working days.

You have a new baby

First-time mothers in Punggol may get KKH nurse-midwife visits. Our after the birth page has the details.

How to Arrange Home Nursing, Step by Step

  1. Write down what is needed. List the tasks (wound care, injections, tube feeding, help with medicines) and how often.
  2. Talk to a doctor. The hospital ward doctor, a Punggol Polyclinic doctor or your GP can advise. AIC says a hospital or polyclinic doctor can submit the application online.
  3. Or go direct. Call AIC on 1800 650 6060 or visit an AIC Link. HNF accepts self-referrals by phone or email.
  4. Prepare for the means test. Subsidies depend on household income per person, or your home’s Annual Value if there is no income.
  5. Use the first visit well. Go through your list of tasks with the nurse and ask any questions you have.
  6. Check the grant. If your family member needs help with daily activities, apply for the Home Caregiving Grant.
  7. Review regularly. Needs change. Tell the provider when things improve or get harder.

What to Have Ready

  • NRIC of the person receiving care, and your own.
  • The hospital discharge summary and any doctor’s memo.
  • An up-to-date list of medicines, doses and times.
  • Household income details for the means test.
  • The main caregiver’s phone number, and a back-up contact.
  • A clear space for the nurse to work, with good light and a place to wash hands.

Costs, Subsidies and Grants

  • AIC home nursing: from $62 per visit before subsidies. Subsidies for Singapore Citizens and PRs are means-tested, and 2026 subsidy levels use 2025 Annual Values. A medical social worker can help if fees are a worry.
  • Medical Escort and Transport: from $40 per round trip before subsidies, means-tested, for Singapore Citizens and PRs. See AIC’s escort page.
  • NTUC Health home nursing: from $10.95 per visit, with government subsidies covering up to 95% of the fee depending on eligibility.
  • HNF: says it provides full waivers of charges using its charity dollars. Ask whether your family qualifies.
  • SKH@Home: priced similar to or lower than a normal ward stay, with subsidies, MediShield Life, Integrated Shield plans and MediSave usable.
  • Home Caregiving Grant: $600 a month if household income per person is $0 to $1,500 (or Annual Value up to $21,000 with no income), $400 for $1,501 to $3,600, and $200 for $3,601 to $4,800. The person cared for must be a Singapore Citizen or PR who permanently needs some help with at least three of six activities of daily living. Apply online with Singpass via AIC’s grant page, or email apply@aic.sg.

Neighbour’s Tips for Home Care in Punggol

  • Ask before discharge, not after. It is far easier to arrange help while the ward team is still involved.
  • Know the weekday limits. NTUC Health’s private-nurse visits are weekday daytime only, with no nights or public holidays.
  • Save HNF’s evening line. Its emergency nursing line runs Monday to Friday, 5pm to 9pm, with last calls at 8pm.
  • Watch for scams. AIC says its staff will never ask you to transfer money over the phone. If a call feels wrong, hang up and ring ScamShield on 1799.
  • Keep a care notebook. One notebook by the bed for medicines, visits and questions helps every nurse and family member stay on the same page.
  • Let HealthHub help. Our guide to HealthHub caregiver access explains how to help an adult family member through the app.
  • Look after the carer. Our caregiver wellbeing and respite guide is for you, not the patient.
