To get a hospital medical report in Punggol, ask the hospital that treated you. For Sengkang General Hospital, KKH and the other hospitals on HealthHub’s list, you can request it in the HealthHub app with Singpass. Allow 4 to 6 weeks, and check first whether a cheaper doctor’s memo or duplicate record will do.
Insurers, employers and schools tend to want paperwork just when you have the least energy for it. A complete request the first time saves weeks.
This page is part of eduKate Punggol’s When You Need Help in Punggol, our practical guides for life’s occasions, big and small. It covers how to request a hospital medical report, insurance form, doctor’s memo or duplicate record from Sengkang General Hospital, KKH and SingHealth Polyclinics–Punggol, with forms, fees, waiting times and who can apply.
Quick answer
- Where to ask: the institution that treated you. HealthHub says you can only request reports from institutions where you received treatment.
- Quickest route: HealthHub app > Services > Health records > Medical reports/certificates requests. Sengkang General Hospital (SKH) and KKH are on its list; NUH, TTSH and the polyclinics are not.
- Waiting time: about 4 to 6 weeks for a medical report or insurance form at SKH and KKH. Duplicate records take 1 to 2 weeks at KKH.
- Cost: an ordinary medical report is S$141.70 at KKH and $122.55 at SingHealth Polyclinics for Citizens and PRs. SKH’s table, dated 1 January 2023, shows S$121.00 (check live).
- Cheaper options: a KKH doctor’s memo is S$21.80 and a polyclinic memo $6.20. Duplicate discharge summaries or MCs are S$12.00 at KKH, and S$12.00 in SKH’s 2023 table (check live).
How to Request a Hospital Medical Report
These steps work for most requests. The hospital pages differ in the details, which are in the table and situations below.
- Ask exactly what is needed. Is it a full medical report, the insurer’s own form completed by the doctor, a doctor’s memo, or just a copy of a discharge summary or MC? Ask the insurer, employer or school before you order the most expensive one.
- Check who can apply. Usually the patient. A minor needs a parent’s or legal guardian’s consent, and insurers can apply with the patient’s consent.
- Gather the documents. SKH and KKH want the consent form, a copy of the patient’s NRIC (front and back), any insurance form and supporting documents. Incomplete requests are delayed.
- Submit through HealthHub if you can. In the app, tap “Make new request”, choose the institution, accept its terms, then select the visit or admission date, department, report type and purpose. Upload one supporting file if needed, add your contact details and choose registered mail or download on HealthHub.
- Pay. HealthHub takes Visa and Mastercard credit cards and debit cards. SKH says requests are processed only once the documents, consent or insurance form and payment are all in.
- Track it. On HealthHub, check your transaction history. SKH warns of delays if the patient has upcoming appointments, is warded, or the doctor is on leave.
- Watch your inbox. KKH sends reports by encrypted email by default and does not post hard copies.

Which Situation Fits You
It is for an insurance claim
Ask your insurer whether it needs a full medical report or its own claim form completed by the doctor, and send that form with your request. Insurers can also apply directly with your consent. At the polyclinic, an insurer needs an official letter or your signed authorisation.
Your employer or school only needs proof of a visit
A doctor’s memo or a duplicate MC may be enough, at a fraction of the price. For the MC itself, our digital MC guide shows where to find it, and our school attendance and MC guide covers what schools accept.
The report is for your child
At KKH, use HealthHub or the HealthBuddy app with Singpass. Without Singpass, email KKH’s “Consent for Release of Medical Information” form and documents; KKH sends payment instructions within 3 working days. For patients under 21, include a copy of the birth certificate and one parent’s NRIC, front and back. The polyclinic also needs a parent’s or guardian’s consent for anyone under 21.
The patient has passed away or cannot decide for themselves
This is a hard time to be filling in forms, so take it slowly. SKH accepts a request from the appointed estate representative. If there is none, all the immediate next-of-kin apply together through a Letter of Undertaking, with the death certificate and proof of relationship.
You do not use Singpass
SKH accepts email (with the consent form, NRIC copy and supporting documents; it replies within three working days), Form.SG, or post to Health Information Management Services, SKH, 110 Sengkang East Way, Singapore 544886. At the polyclinic, fill in the “Medical Report / Duplicate / Memo with Endorsement Request” form, also available at the registration counter, and pay when you submit it.
You were treated at NUH or TTSH
They are not on HealthHub’s request list, so contact the hospital directly. We have not checked their fees for this guide. Changi General Hospital is on the HealthHub list.
Hospital Medical Report Fees
Fees as published by each institution. KKH’s and the polyclinics’ include GST. SKH’s table is dated 1 January 2023, before GST rose to 9%, so check live before you rely on it.

SKH’s 2023 table also lists S$222.00 for a permanent disability claim form (check live). The polyclinics charge $7.50 for courier delivery, and $133.60 for a report requested by an insurer. KKH says there are “no refunds or cancellations once an application has been processed”. On HealthHub, fees are set by each institution.
Neighbour’s Tips
- Start early. Four to six weeks is the hospitals’ own guide, so request the report as soon as you know an insurer or employer will want it.
- Ask whether a memo will do. At KKH, a memo is S$21.80 against S$141.70 for an ordinary report.
- Lost a discharge summary or MC? A duplicate is S$12.00 at KKH (SKH’s 2023 table also shows S$12.00; check live), and takes 1 to 2 weeks at KKH.
- Send everything at once. SKH processes a request only when the documents, consent form and payment are all in, so a missing NRIC copy holds everything up.
- Give the visit date and the condition you need covered. The polyclinic asks for both, and it helps the doctor write the right report.
- Check your email, including the junk folder, when a KKH report is due, as it arrives encrypted rather than by post.
If It’s Urgent or Gets Worse
A report request is never an emergency. If someone is seriously unwell, call 995. For non-urgent health advice, the NurseFirst helpline is 6262 6262.
Watch for scams around payments. SingHealth Polyclinics says it will never ask you to transfer money over a call. If a caller asks you to pay for a report by phone, hang up and check with ScamShield on 1799.
