How to Withdraw School Dental Consent: A Punggol Parent Guide

Three students in school uniforms work through open books at a classroom table, with textbooks and stationery nearby and study notes on the whiteboard behind them.

To withdraw your child from school dental services, HPB says to open the Child Consent Portal and select “No” for the consent status under the child’s school dental services. This withdraws the consent.

Punggol parents should check both the child’s name and the service before changing the record. School dental consent is a specific choice; do not assume it is the same as every health or vaccination consent.

If your child already has an appointment or ongoing follow-up, clarify those arrangements with the dental team separately. A consent-status change should not be treated as confirmation that another booking has been cancelled or alternative care arranged.

CHAPTER 1 OF 4

Open the official consent record

Use the HPB Child Consent Portal. HPB’s portal access guide also explains access through HealthHub.

Before changing anything, confirm that you are viewing the correct child and the school dental service. Read any current instructions displayed in the portal.

CHAPTER 2 OF 4

Change the dental consent status

The HPB withdrawal instructions specify selecting “No” under the child’s school dental services. Follow the portal’s prompts and check the resulting status.

If you cannot find the service or the change is unclear, use the official help route. Do not guess by changing an unrelated form.

CHAPTER 3 OF 4

Clarify any existing follow-up

Contact the relevant dental team if you need to understand how the change affects a planned visit or current care. State the child’s existing appointment or referral details.

If the reason is that your child uses another dentist, tell the providers about relevant care arrangements where needed. Keep the records clear so everyone knows which next appointment the family is following.

CHAPTER 4 OF 4

Keep dental care organised

Agree who will arrange your child’s dental appointments and retain the current provider’s instructions. Withdrawing from one service is separate from planning the child’s ongoing care.

If you later want to use the school service again, ask HPB or the school dental clinic about the current process. Do not assume that a previous booking remains available after a consent change.
